Medical Release Form
By official rules in Egypt you must complete a Self Declaration Medical Form before being allowed to dive from any liveaboard.
Please download the Medical Release Form here, and read it carefully. If you do have any of the medical conditions listed, you will need a signed clearance from a doctor, or we can not allow you to dive!
If in any doubt, you should contact a physician to receive an exemption before your embarkation.
It is the responsibility of each passenger to ensure that any necessary medical clearance is obtained before booking the holiday, there will be no refund if you cannot dive due to medical problems or a missing medical statement.
